Distinguished Service Award Nomination Form

Who are you Nominating for the Distinguished Service Award?

In 1973 the National Executive Council approved the establishment of a Distinguished Service Award to be awarded to individuals, in recognition of and appreciation for distinguished service to the Association. The Distinguished Service Award is the highest award presented to an individual by the Association. Two awards may be presented each year. Nominations should normally come from amongst those who have previously been awarded an Honorary Lifetime (Regular) Membership Award. However, any Canadian citizen of excellent character, who has rendered unique and distinguished service to the Association, or to Canadian Aviation in any of its aspects, is also eligible for the award.
