F/O Stewart had completed over thirty trips and had begun his second tour of operations when his Lancaster aircraft #JA 915 crashed at Kennighall, near Thetford, Norfolk, England. F/Os W.C. Gordon D.F.C., C. Foderingham D.F.C., FS H.R. Ross, and two of the crew, not Canadians, were also killed. Halifax aircraft #MZ 808 was shot down over the Dam between Agerness and Helnas Penninsula on the south-west coast of Denmark during a night mine- laying operation at Kiel, Germany. F/L J.A. Morgan, F/O J.E. Clerc, FS J.W. Moffat, and Sgt R.F. Young (RAF) were also killed. These five airmen were buried in the same collective grave. Two Canadians, F/Os G.H. Gill and P.E. Marchildon, were taken Prisoners of War. Pilot Officer Wireless Operator Air Gunner Stewart is buried in the Churchyard at Sonderby, Island of Fyn, Denmark.

Halifax aircraft #W 1155 crashed in the field adjoining north side of the Leeming aerodrome, Yorkshire shortly after take-off for an air test. Sgts W.J.W. Wiseman (RAF), S.V. Duvall (RAF), J.A. Carter (RAF), and F/O Mutter (RAF) were also killed. One Canadian, P/O D.A. Street, was injured and was later Killed in Action. Thunderbolt aircraft #KL 174 dropped 2 x 1,200 lb. containers of napalm and failed to recover from the evasive action F/O Stewart had to take to avoid a collision with the rest of the Sqdn. He was killed when his aircraft crashed at Myingyan, Burma. F/O Stewart had previously flown Thunderbolt aircraft when assigned to #30 Squadron.
